Instructions-Lodging Facility Bed Tax Form:  This is an instruction sheet to be used by lodging facilities to help them prepare & complete the quarterly bed tax form.

Lodging Facility Bed Tax Form:  This is the quarterly form that is due to Madison Village to be completed by the lodging facility.

Info Explaining Local Bed Tax:  This is a form that explains the nature of the Village Bed Tax and a summary of important information about the tax.

Local Bed Tax Exemption Form:  This form should be completed by guests that are exempt from sales & use tax and stay at a Village lodging facility. Along with this form, the guest should also submit a State Exemption Form to the lodging facility.   The facility will need these when filling out their quarterly tax returns, and keep them on file for potential audit purposes of those returns.
